Compounding needs its own explanation.

Ask about the actual preparation and who makes it.

With compounding, the medicine is prepared by a doctor or pharmacist. It comes from that preparation, rather than as an approved finished product. Read the FDA’s explanation of this work; FDA means Food and Drug Administration.

Larger sites can also prepare batches of medicines for clinics and hospitals. These sites can register with the FDA as outsourcing facilities; below, we explain how their rules differ from a pharmacy’s.

For compounded products, FDA does not review quality, effectiveness or safety before marketing. They lack FDA approval, including when made by a well-run pharmacy; this position is dated September 26, 2026.

Can sermorelin be prescribed this way?

The full ingredient name here is sermorelin acetate. Acetate identifies the chemical version used in Geref, the former approved drug; it is not a word for any version of sermorelin.

A state-licensed pharmacy may have a way to prepare that version for you. Your prescription must be valid and identify you by name. This is a reading of federal documents checked September 28, 2026, rather than a specific FDA permission naming sermorelin.

Why may it be possible? The FDA ingredient policy provides a way to use an ingredient from an approved drug. FDA still counts sermorelin acetate that way in its ingredient record, even though Geref is no longer sold.

What else must the pharmacy check?

The company producing the raw ingredient must register with FDA. A valid certificate of analysis must accompany the ingredient; this document reports checks on it. If an official ingredient standard applies, its published requirements for the ingredient must also be met. The pharmacy also needs your valid prescription with your name on it; these conditions were checked September 28, 2026.

Federal rules also limit regularly making copies of medicines already being sold. FDA does not treat a drug that has been discontinued and is no longer sold as commercially available; that matters for Geref, as of September 28, 2026.

State pharmacy rules can add limits, and this route does not make the resulting product FDA-approved, as of September 28, 2026.

A possible pharmacy route. No FDA product approval.

Ask the pharmacist to explain how each condition is met.

Different rules for batch-making sites

The pharmacy rules above are often called 503A, their section number in federal law. Section 503B is a different set of rules for outsourcing facilities, the sites that register with FDA to prepare batches. For batch preparation, the site need not hold a prescription bearing your name; these rules were checked September 26, 2026.

These batch-making sites must follow federal factory quality rules. FDA inspects them according to risk. Registering the site gives neither it nor its products FDA approval; these rules were checked September 26, 2026.

For these sites, sermorelin acetate is still being evaluated as an ingredient. As of September 28, 2026, it is absent from the final allowed list for this batch-making route. That list is separate from the pharmacy rules above, so its absence does not cancel the possible pharmacy route.

During the review, FDA’s temporary policy for batch-making sites says it intends to hold back from enforcement over use of this ingredient only for facilities meeting its conditions. This concerns planned enforcement, rather than FDA permission to use it; the policy was checked September 28, 2026.

The sources used here do not give a complete checklist for that temporary policy. Ask the pharmacist to name its requirements and explain in writing how the maker meets each one. A place on a list is not enough to settle whether your particular offer meets the rules.
